ICR – Integrated Care record
focus designed and has had a fully integrated Health & Social Care record called an Integrated Care Record (ICR) which has now been operational for 2 years. It allows our social workers to access GP records / data as well as community health, and vice versa. All based on patient consent. Each Local Authority is unique, and if we can help you work towards what we have achieved and share our experiences / skills – we can offer consultancy support throughout your journey.
focus recognises the national performance frameworks in place for many statutory functions / organisations and how exhausting reports can be with information, and confusing charts. The Performance Wheel will combine both national and local indicators to suit your needs and succinctly report on the ‘story’ and ‘actions’ of your data in an easy read format. and reviews.
Services4Me – Advice & Info
focus designed and committed itself to working with its community to design a local online directory of all health & social care services within North East Lincolnshire. As well as to enhance the Single Point of Access. This has been operational for over 3 years and has now expanded to include an online self assessment, online wellbeing plan, citizen portal and a brand new e-Market place and Online Personal Budget Manager (myOPBM). We can now offer the skills and experience we have gained in achieving this to support those with similar ambitions.
